The Senate of Lübeck established this portable rescue medal with an announcement on 18 December 1918. The rescue medals awarded until then were already donated on 29 July 1882 and were not portable. The medal was awarded for saving human lives using one's own life. Medals of the first form, not worn, were awarded from 1882 to 1918 1 time in gold and 86 times in silver. The medal was awarded only to adults. Minors received monetary rewards or engraved pocket watches. Upon reaching the age of majority, the rescuer was awarded a medal with an award certificate. The provisions for awarding remained the same for the portable medal. The loaned ones were recorded in the state archive. Medals awarded by that time could be exchanged for a wearable medal. The medal was not refundable.
The medals were made at L. Ostermann in Berlin. By 1928, the company had delivered 70 medals. In total, the medal was awarded 55 times. Among the detainees were 9 foreigners. Crew members of the Danish steamer "Estonia" and Prussian.
