Women, virgins, as well as men were awarded for special merits in the field of voluntary assistance in war and in peacetime. Queen Olga was a defender of the Württemberg Red Cross Medical Association. On June 16, 1889, the Queen donated the "Karl Olga Medal" in bronze and silver. After the death of the Queen on February 17, 1893, the medal was attached to the Order of Olga and awarded as the lower level of the order. Since October 18, 1905, the possession of the Karl Olga medal in silver was a prerequisite for awarding the Order of Olga.
When awarding men, the cross was equipped with a transversely standing ear, for women the cross was awarded a parallel ear for a female loop.
