The medal was established in 1881 without a charter by Grand Duke Ludwig IV. donated. In general, persons who distinguished themselves with outstanding merits in front of the Hessian grand ducal house were awarded. The medal was awarded in silver and bronze. Both medals are very rare. The silver medal was awarded only 10 times, and the bronze medal was awarded 14 times. The stamps were made by the Darmstadt medalist J. Ries. The first awards took place on April 25, 1884. April 25 was the birthday of Grand Duchess Alice, who died in 1878. On the same day, Alice medals were awarded. Among the silver medal winners were 2 women. Both women held posts at the Alice Women's Club. Marie Winter was chairman of the Alice Hospital and was awarded in 1894, Mrs. Feist-Lorch as chairman of the association in Mainz was awarded in 1895. Other speakers came from spheres such as the military or foreign rulers and dignitaries.
